Thank you for taking time to respond to our vendor survey. The survey consists of just nine questions--and it is ANONYMOUS. We WILL NOT be able to trace your responses back to you or your email address. It should only take a few minutes of your time to answer these questions--and the information you provide will help us improve the market for next year.

Your feedback is also needed to help us with final reports for two grants we received this year. The first one was from the Illinois Department of Agriculture and was used for our Market to Menu program with WGLT radio and WMBD television. The second grant was from Experimental Station--and it was used for our Link Double-Value program--which allowed us to give double tokens for Link card purchases up to $25 per day. Together with the Link card, this program added over $8,000 in sales revenue to the farmers' market this year.

When promoting the market, it would also be useful to know total gross sales in dollars -- so we are asking for that information as well. We know this is sensitive information--and that is why we have set up the survey so that YOUR RESPONSES ARE ANONYMOUS. Because responses are anonymous, we are hoping you will feel comfortable sharing sales information with us.
